Shocking that this can and does happen pretty regularly in F1 (at least pit stop problems do...)
The teams are under too much pressure to achieve a 2-second stop that simple checks/procedures are not being done - a few times in the German GP i noticed the 'traffic light' go green just before the mechanic had their hands out of the way - in Red Bull's case not once did the mechanic involved signal he had finished, he was still fiddling with the tyre and couldn't believe it when the car started to move - who controls the green light to tell the car to move?!
There should be a minimum stop time or something, I think the pedulum between safety and speed has swung too far away from safety.
